Dear Colleagues,
I am pleased to recommend this instrument, which is very useful for all routine treatments and is not confined to onychocryptosis cases.
This is because the tool can be used to gradually, and gently, shave back the lateral nail edge and, in the case of abruptly-curved and involuted nails, the lateral nail under-edge, thus creating the sulcal space to allow you the visibility to accurately access sulcal point-helomas and strip-helomas. This puts the podiatrist in a position to then accurately remove these helomas with a Number 15 or 15C scalpel blade. These helomas are present to varying degrees in nearly all patients that I see. As well as creating greater comfort for patients, the creation of sulcal space alone can be a preventive and pre-emptive treatment to de-escalate the sulcal pressures that may eventually lead to the development of sulcal helomas and onychocrpytosis. Without this tool, routine treatments have a greater level of difficulty in checking for, exposing and removing sulcal helomas. lndeed, it is possible that these helomas could otherwise go unnoticed and untreated. The unique and clever design feature of this tool is that its blade will always be positioned to touch the lateral nail edge or lateral nail under-edge, and always point away from the soft tissue of either the paronychium or nail bed, lowering the risk of causing cuts and patient discomfort during treatment. The tool can also be used initially, to gently feel along the under-edge of an abruptly-curved or “involuted-nail, for-areas that may be sharp, jagged or-uneven, and then be gently smoothed by the shaving action of the blade. I have to add, that in my experience, the tool may sometimes not be optimally effective in cases where there is copious exudate or blood and the nail has become macerated or soggy. ln these cases, the nail will tend to disintegrate and tear as there will not be the required level of dry friction to enable the shaving action to take place. ln addition, the tool can simply be used to: - Tease out onychophosis and other kinds of debris that have become trapped under the nail
- Round off the distal nail corners
