Chicago Medical Supply, LLC was formed in 2011 with a single distribution center and corporate headquarters located in Northbrook, Illinois.

Shipping:
All orders over $150 within continental United States ship for Free via ground shipping services. Orders for cases of Barge Cement Gallons or X-Ray Chemistry ONLY that qualify for Free shipping incur a $15/Case shipping surcharge.
Note: 1Lb is the minimum calculated shipping weight for UPS/FedEx so for lightweight products shipping charge will be the same for 1 box of Band-Aids or 10 boxes. First Class airmail is the most economic shipping option available for lightweight items.
We only charge actual shipping and there are NO FUEL SURCHARGES.
All Hazardous (HAZ in the item description) shipments incur additional $48.00 fee.
Return Policy:
We only sell NEW merchandise. If you change your mind about a purchase, you have 7 days from receipt to email us for return authorization (RA). The product must be in new/unused condition with original packaging. Returns and/or exchanges must be completed within 30 days of original purchase. All claims for incomplete, defective and/or missing accessories or products must be made within 7 days merchandise is received. Exchanges for color, size etc will be handled as new orders. If the original order included Free Shipping, we will deduct the actual cost to ship to you from the refund amount if you return the product. Due to our compliance with Health and Safety laws, many products we sell are not returnable. We truly regret the strictness of this policy, but require it to ensure the hygienic integrity of our products.
If you have any questions you are highly encouraged to call or e-mail us in advance of making a purchase. The majority of the products offered are common items that most customers should be familiar with.
The following is a partial listing of our most popular items that are not returnable: 1. No health or herbal products, gels or lotions, may be returned or exchanged once delivered. 2. Used or worn orthopedic soft goods, supports, braces, foot products are not returnable. 3. Any item showing use/wear
We will only accept returns for items that are 100% complete in original, unopened packaging and in resalable condition. We reserve the right to refuse a return on any product that does not meet these requirements. Any such products will be destroyed.
Please do not return used, damaged or incomplete merchandise. It will only create a difficult situation and we will not refund used merchandise.
